    South Korea is rapidly becoming a super-aged society, presenting urgent challenges in healthcare delivery for older adults. Sarcopenia, frailty, and functional decline often go undetected in traditional clinic-based care. Smart home technologies equipped with sensors, wearable devices, and AI-based integration offer a promising pathway for continuous, real-life monitoring of health and daily living. However, large-scale evidence in Asian aging populations remains limited. The Incheon Smart Home Aging Cohort (ISHAC) was established to collect and manage multidomain, real-time daily living data of older adults across five core service domains: (1) exercise rehabilitation, (2) mental health, (3) well-care, (4) primary healthcare, and (5) home safety. The aim is to identify risk factors for sarcopenia and activity limitations, while developing scalable smart home models for aging-in-place Incheon Smart Home Aging Cohort. This prospective cohort recruits community-dwelling older adults (≥55 years) in Incheon. Participants undergo standardized assessments every three months at the Gachon University AAL Smart Homecare Evaluation Center, including physical function (SPPB, 3D gait analysis, isokinetic dynamometry, grip strength), cognitive and mental health (MMSE, MoCA, GDS, fNIRS), well-care (nutrition, oral health, sleep, chronic disease status), community-based primary health (BIA, vital signs, bone density), and safety/emergency preparedness (fall risk assessment, smart home acceptance). In parallel, continuous sensor-based monitoring in the smart home environment captures data on mobility, daily activity, sleep, and environmental safety. The ISHAC will generate longitudinal multidimensional datasets linking sensor-derived daily living patterns with standardized health outcomes. This will enable identification of early indicators of sarcopenia, frailty, and mental health decline, while validating smart home–based digital biomarkers across the five service domains. By integrating smart home infrastructure with multidomain health monitoring, ISHAC provides real-world evidence for designing next-generation, community-based healthcare solutions in super-aged societies. This cohort is expected to inform precision interventions, enhance quality of life, and support policy development for scalable aging-in-place services in Incheon and beyond.
